Essential Functions Statement(s)

• Assist the resident with personal cares as identified in the assignment sheet/client care plan. These cares may include all or some of the following: assisting with dressing, grooming and oral cares; shampooing of hair; assisting with showering or bathing; rubbing with lotions; fingernail cares and foot cares if instructed to do so by an RN; redirection of a confused client, and other care as assigned.
• Assisting the resident in the use of the commode, urinal, toilet and with adult incontinence products as assigned.
• Assisting with ambulation and transfers, including proper use of a transfer belt and assistive devices and proper body mechanics to avoid injury.
• Assist in use of hearing aids, glasses and other adaptive devices for residents.
• Medication reminders, assistance or administration as delegated by RN.
• Prepare meals or snacks as directed, following special meal plans if instructed.
• Take and record temperature, blood pressure, pulse and respiration as instructed. Report changes or concerns to RN.
• Notify RN of any complaints or concerns raised by the resident or the resident's family or representative.
• Help each resident to reach and maintain his/her highest level of performance and independence. Must promote clients' independence, not dependence by offering choices and fostering self-help skills.
• Establish rapport with residents, lead resident in activities as assigned and provide socialization and companionship to residents while performing assigned tasks.
• Assist with services or treatments as delegated by the RN according to the resident's individualized care plan or with therapy tasks as assigned by a licensed health professional.
• Observe resident and report to the LPN or RN changes in the resident's physical and emotional condition, such as change in attitude, appearance loss of appetite, weight, etc.
• Assist co-workers with daily responsibilities when needed.
• Organize and prioritize assigned work during scheduled shift to complete assigned tasks.
• If working in the community, responsible for taking adequate supplies or equipment to the residents' home that are needed to complete the assigned tasks.
• Document services provided accurately and consistent with agency policies and complete all required paperwork in a timely and legible manner.
• Attend required in-service training- at least 8 hours for every 12 months of employment as well as staff meetings and other agency events.
• Follow agency procedures for universal precautions for infection control when performing assigned tasks.
• Maintain a clean environment for clients. Dust, vacuum, mop and other household chores as scheduled and assigned.
• Use validation techniques and/or appropriate diversion activities when dealing with residents with dementia.
• Follow agency policies regarding safety rules and requirements and report any issues or concerns about your safety or any injuries immediately to the LPN or RN.

POSITION SUMMARY

This position requires the employee to prepare meals for LB Woodland Lodge. Take and record temperatures of coolers/freezers and food items. Ensure proper staffing for the meal period. Prepare food items based on daily censes and in accordance to dietary standards and recipe compliance for Woodland Lodge. Maintain a clean and orderly workplace and ensure other staff follows. Assist manager with inventory, purchasing, scheduling, and training as assigned. Ensure that the storage areas are well organized and clean to company standards. Ensure proper cleaning is enforced throughout the department following wash and sanitize standards. Keep manager up to date on equipment and safety issues. Must obtain ServSafe certification within first 6 months (or when class is available locally).

 POSITION SUMMARY

This position will primarily be a Part-Time Evening Shift (5p - 8p); approximately 6-9 hours (2-3 days per week including possible weekends).  Position will require employee to be in proper uniform daily, stock station with required amount of supplies, assist with minor meal production, assemble resident meal plates, clean station after meal period, assist with washing and storing all dishes and service ware for next meal period, take and record all necessary temperatures accurately and consistently, assist with overall cleanliness of the Food Service Department including cleaning floors, removal of garbage, anything else needed to help department work efficiently and effectively as assigned by supervisor.

LB Homes is looking for an enthusiastic and able Part-Time Maintenance Assistant primarily for our Broen Home facility.  Hours are approximately 7:00am to 3:30pm every other weekend (approximately 16 hours per week).  

POSITION SUMMARY

Duties involve general maintenance tasks as well as grounds work including weeding, spraying, watering, etc.  Position is responsible for taking care of garbage, locking and unlocking doors, general cleaning and maintenance, and for performing work to keep facility and buildings in good condition.  Other duties may involve routine maintenance and repair of electrical or mechanical equipment, installing and assembling equipment, painting, cleaning, repairing, and maintaining all aspects of the buildings and grounds. In addition, position is responsible for transporting nursing home residents and their belongings as needed.

• Recognizes an emergency situation and responds to it, providng leadership and direction to staff. Acts in the role of Incident Commander until additional leadership staff arrives.  
• Monitors use of facilities and staff time to ensure effective use of resources. Assesses the need to adjust staff, equipment and services and brings recommendations to the RNUC's, ADON, SSU Manager and the DON.  
• Supports and participates in the onboarding and training activities for new LB Homes employees. Participates in staff training and continuing education in the nursing home.  
• Implements care delivery systems in the Nursing Home.  
• Maintains an attitude of respect and dignity toward all individuals: families, staff and residents. Communicates effectively and in a courteous manner.  
• Completes clinical assessments and care planning for all residents as needed according to resident conditions.  
• Ensures all staff's compliance with LB Homes Employee Handbook.  
• Collaborates with the RNUC's, ADON, SSU Nurse Manager and DON to direct, supervise, evaluate and document performance of all staff working in the nursing home.  
• Leads, manages and holds each staff accountable to ensure compliance with resident care plans, facility policies and procedures and LB Homes Core Values.  
• Performs and supervises activities and assignments with a commitment to promoting employee and resident safety.  
• Assures that resident care and services are performed following infection control standards, policies and procedures.  
• Is responsible for compliance with the reporting requirements of the Vulnerable Adult Act including the investigation and immediate reporting of suspected maltreatment.  
• Maintains accurate electronic health records which reflect care and services needed and provided and to store and process data and produce reports within HIM standards and HIPPA requirements.  
• Completes tasks and assignments as listed on the position task listing.
